Getting In Touch

When you call for an appointment, you will be connected to a member of our professional, experienced customer care center team. They will ask for information regarding the purpose of your visit. This will assist us in scheduling the best time and length of your appointment. We will also determine the best office location that is most convenient for you.

Your appointment can be scheduled either by your doctor’s office or by yourself. In either case, you will need a Referral Form (Requisition) from your doctor, and you may need pre-authorization from your insurance company.
