How Often Do These Prostate Symptoms Affect You?
Whether you start with your urologist or with our consultation, we’ll help you understand your options and next steps. We’ll also work closely with your urologist and PCP to define the care plan that’s right for you.
Complete the survey below and click submit. Our team will review your responses and call you to discuss whether you may qualify for a PAE consultation.
UNDERSTANDING YOUR SCORE
Your answers give you a score between 0 and 35. This score helps measure how much your urinary symptoms may be affecting your daily life.
- 0–7 points: Mild prostate symptoms
Your symptoms may not significantly impact your quality of life. - 8–19 points: Moderate prostate symptoms
Your symptoms are likely interfering with your daily activities. It may be time to explore treatment options. - 20–35 points: Severe prostate symptoms
Your symptoms are significantly affecting your quality of life. Advanced treatment, such as Prostate Artery Embolization (PAE), may be appropriate.
Note: This survey is a screening tool, not a diagnosis. The questions are based on the American Urological Association (AUA) Symptom Score, a trusted tool used by physicians to determine the severity of your symptoms, which is one step toward obtaining a diagnosis of an enlarged prostate. Regardless of your score, if your symptoms are disruptive, we encourage you to speak with your primary care doctor or call us to see if you qualify for a PAE consultation.
